Rectangular off white piece of paper with brown/gray ink scalloped edged rectangle in center. Text across bottom reads "POLSKA KRAJOWA KASA POZYCZKOWA." On verso is emblem surrounded by text in circle that reads " TYSIECY MAREK POLSKICH PIEC" with "5000" on both sides. On lower left "III Serja AO" and on right "N 127372."
