The collection was donated to the United States Holocaust Memorial Museum by Peter Leers in 2002.

Bi-folded rectangular form constructed of gray designed paper; handwritten and printed text; interior reveals two finger prints; black and white photographic image of Lea Abel adhered to back cover; white rectangular paper insert with printed and typed text on recto and verso, issued to Lea Abel, and titled "BEWIJS/VAN/AANMELDING,.."; plastic sleeve for holding card.
